Douglas County, Oregon (June 30, 2023) – The Oregon State Police has identified 35-year-old Raymond Louis Eby Jr. of Molalla and 51-year-old Shawna Michelle Coolidge of Florence as the two persons killed in a head-on collision on Wednesday afternoon in Douglas County.

The accident happened on Highway 101 near milepost 208 at about 12:10 p.m. on June 28.

Police said Eby Jr. was heading southbound in a silver GMC Sonoma when he drifted into the northbound lane and crashed head-on into an oncoming white Toyota RAV4 driven by Coolidge.

The crash caused the Sonoma to roll over and overturn on its roof, blocking the northbound lanes. On the other hand, the Toyota swerved off the roadway and halted in a small embankment.

Eby Jr. and Coolidge were declared dead at the crash site.

A three-year-old child riding in the Toyota was critically injured and was taken to Lower Umpqua Hospital before being transferred to Sacred Heart Riverbend in Springfield.

Authorities blocked the highway for approximately one hour following the collision.

The incident remains under investigation.
